Before diving in Komodo — or anywhere in the world — you must complete a medical self-assessment questionnaire. Depending on your answers, you may need a physician's clearance before being allowed to dive.
Every diver is required to complete the RSTC (Recreational Scuba Training Council) Medical Statement before diving. This is a standardised form used worldwide by PADI, SSI, and all other agencies. You will be asked to complete it at check-in with your dive centre.

If you answer "yes" to any condition on the questionnaire, you will need a signed letter from a physician stating that you are fit to dive. Ideally, this should come from a doctor trained in dive medicine. Options include:
Divers over 45 years old are recommended to have a medical clearance even if they answer "no" to all conditions. Some operators require this. An annual check-up with a dive medicine physician is good practice for all divers over 45.
Tip: Complete the RSTC medical questionnaire at home weeks before your trip. If clearance is needed, arrange it in advance — being turned away at the dive centre on your first morning is a disappointing way to start a holiday.
